I cannot imagine that a US physician would not do all of the above as well as a lot more. X-rays and/or MRI studies might also be useful. I also suspect that your problem is not orthopedic in nature. I am not a physician, but with some answers to the questions above, I might have a shot at knowing what is going on. But, my knowing is not going to help you.
